Burns brings two decades of nonprofit experience to leadership role

Nazareth Home is excited to announce that Carrie Burns has been hired as its new director of development. Burns will lead the organization as it further develops the Nazareth Homes Foundation. 

Mary Haynes, president/CEO of Nazareth Home, said Burns adds great leadership to the team, bringing decades of experience and a passion for elder care to the role.

“Carrie comes to this role with a commitment to serve and a gift of cultivating donor relationships,” said Haynes. “We’re very excited to have Carrie on our team. Our board and leadership are excited to have her join Nazareth Home and know that her longstanding passion for working in the nonprofit sphere and her devotion to elder care will add great depth to our mission and ministry.”

Burns brings more than 20 years of nonprofit experience to the position, working in leadership roles with nonprofits including the Kentucky Humane Society, St. Paul United Methodist Church, the National Multiple Sclerosis Society (Ky Chapter), and more. She most recently served as vice president of development for Wesley Manor Retirement Community.

Honorable Order of Kentucky Colonels Award 314 Grants Totaling $3.1 Million

Nazareth Home an Honored 2022 Recipient

Nazareth Home is proud to announce it recently received a grant from the Honorable Order of Kentucky Colonels (HOKC) in the amount of $20,000 to purchase new kitchen equipment for its Clifton Campus located at 2120 Payne St. 

With two campuses in Louisville, Nazareth Home enriches the lives of its patients through personal care, memory care, rehabilitation-to-home, and long-term care programs. Nazareth Home believes in “person-centered care,” which means meeting the unique needs of each elder by empowering them through personal choices.

“We are appreciative of our partnership with the HOKC and incredibly grateful for the generous funding they have provided,” said Craig Jennings, Nazareth Home’s Chief Operating Officer and Clifton Campus Administrator. “Our dietary team works diligently to ensure that the individual nutritional needs of each elder are met on a daily basis. Having new kitchen equipment helps them in this pursuit to provide a nourishing, well-balanced diet for residents.”

HOKC will award $3.1 million in grants supporting 314 non-profits, impacting more than 3.9 million Kentuckians. Grants are made possible through donations from contributing Kentucky Colonels from throughout the Commonwealth and around the world who chose to exercise this honor in a meaningful way.

“Through extremely generous individuals, we were able to increase our regular grant impact for the Good Works Program by over $1 million,” said Gary Boschert, Commanding General for HOKC. “This year, we are awarding $3.1 million, the most money we have ever been able to spend on the program.”

Nazareth Home announces hire of new Chief Financial Officer

Nazareth Home, a long-term care and rehabilitation organization, is pleased to announce that Sarah Askin, CPA has been hired as its new Chief Financial Officer. Askin will play an integral role in planning and forecasting the overall financial vision of the organization.

With more than 20 years working in public accounting, Askin brings significant experience from previous roles where she provided assurance and tax services to both small businesses and nonprofit organizations.

“We’re very excited to welcome Sarah to the Nazareth Home family,” Nazareth Home President/CEO Mary Haynes said. “Sarah’s expertise will enable her to make an immediate impact as we continue to position Nazareth Home for a successful future in the ever-changing landscape of senior care. She is an excellent addition to our leadership team.” 

“I’m honored to join such a reputable organization in the Louisville community,” Askin said. “Nazareth Home’s mission of providing compassionate care for seniors aligns with my core values, and I look forward to contributing to the long-term growth and success of the organization.” 

Askin holds a Bachelor of Science in Business with a concentration in accounting from Indiana University Southeast. She is a member of the American Institute of Certified Public Accountants and the Kentucky Society of Certified Public Accountants.

Nazareth Home Receives 2022 Customer Experience Award from Pinnacle Quality Insight

Nazareth Home has received Pinnacle Quality Insight’s 2022 Customer Experience AwardTM, qualifying in the category of skilled nursing facility. The award signifies Nazareth Home’s continued dedication to providing best-in-class healthcare services for seniors. 

The customer satisfaction report by Pinnacle Quality Insight is designed to measure residents’ satisfaction with regard to their quality of care and quality of life. 

The Highlands and Clifton Campuses both received awards for skilled care. The Highlands Campus was also awarded for its personal care program. To qualify for the Pinnacle Customer Experience Award™, Nazareth Home had to score in the top 15 percent of the nation across a 12-month average. Out of 2,700 care providers and 150,000 customer interviews, only those facilities that scored in the 85th percentile or higher received an award.

President/CEO of Nazareth Home, Mary Haynes, described receiving this recognition as a great honor, stating that the organization’s priority is keeping the residents safe, healthy and engaged.

“We are thrilled to be awarded for providing an environment that engages employees to deliver award-winning care where elders can thrive, and families can feel confident their loved one is receiving the best care,” said Haynes. “We are making our mark on the national scale by providing exceptional person-centered care to the elders we serve at both our locations.”  

Throughout its long history of serving the community, Nazareth Home has placed a strong emphasis on ensuring that the individual needs of every resident are met. A sampling of Nazareth Home’s residents and their families participated in monthly telephone interviews that included open-ended questions, as well as opportunities to rate Nazareth Home in specific categories. Every month, Nazareth Home gathers its real-time survey results to gain a better understanding of the residents’ needs and make enhancements when necessary.

Collaborative program adds an extra layer of individualized care for long-term care residents facing complex medical issues

Nazareth Home, in collaboration with Pallitus Health Partners, a part of Hosparus Health, has announced the implementation of a new complex disease management program known as HELD — Helping Embrace Life Decisions.

The HELD program supports Nazareth Home residents who have extreme or complex medical situations that require the attention and expertise of multiple health care providers. HELD brings together experts in the fields of geriatrics and complex disease management to develop an individualized plan that focuses on the unique healthcare needs of the Nazareth Home resident. It emphasizes closer monitoring of patients and engaging earlier in symptom control so individuals can discuss their goals and options, as well as identify a meaningful healthcare plan that can improve their quality of life for a longer period of time.

“The HELD program aligns with Nazareth’s Home commitment to be forward-thinking in how we deliver compassionate care, especially to those who are facing more complex situations with their health,” said Mary Haynes, president/CEO of Nazareth Home. “This is really the next generation of care to empower the elder with complex diseases to have more control over their goals, their care options, and ultimately the plan on how they want to live out their best life.”

The program includes a team of consistent providers, including staff members from both organizations, along with Dr. Tyler McQueen, a geriatric medicine specialist with Pallitus Health Partners. McQueen meets with Nazareth Home residents on a weekly basis concerning quality of life decisions during their critical time of need.

McQueen is part of Hosparus Health and the Central Louisville clinical team. He earned his medical degree from the University of Virginia School of Medicine and completed his residency in internal medicine at Wake Forest School of Medicine in North Carolina. He then completed fellowships in geriatric medicine and hospice and palliative medicine at Wake Forest. He is board-certified in internal medicine, geriatric medicine, and hospice and palliative medicine, and is a member of the American College of Physicians.

“This partnership, one of the first of its kind in our area, is an innovative, person-centered approach to caring for those with serious illness. As a provider of palliative care, we offer an extra layer of support that is consistently shown to improve quality of life for those with the most complex or advanced health conditions. We remain committed to meeting patients where they are and finding new ways to help them make the most of each day. Our collaboration with Nazareth Home is a natural extension of our mission,” said Bethany Snider, chief medical officer of Pallitus Health Partners.

Pallitus Health Partners offers comprehensive palliative care for serious illness in Kentucky and Indiana. Specialized services include chronic symptom and medication management, disease education and care coordination.

Nazareth Home earns U.S. News & World Report High Performing rating

Nazareth Home earns U.S. News & World Report High Performing rating

On Nov. 9, Nazareth Home’s Highlands Campus on 2000 Newburg Road in Louisville earned a U.S. News and World Report High Performing rating in the area of short-term rehabilitation. This distinction places the 118-bed facility among the elite 13% of nursing homes that received a “High Performing” status. 

The U.S. News Best Nursing Home ratings are based upon data from Nursing Home Compare, a program operated by the Centers for Medicare & Medicaid Services. The Short-Term Rehabilitation rating began in 2018 and assesses the quality of care provided for patients hospitalized after a surgery, heart attack, stroke, injury or similar condition. The rating is based upon U.S. News’ assessment of 10 quality measures, which focus on staffing, medical outcomes, resident complaints and processes of care. 

This year, some new requirements came into play for homes to earn a High Performing rating, including a requirement that federal Centers for Medicare & Medicaid Services must list the home as having a COVID-19 staff-vaccination rate of at least 75%.

Of the more than 15,000 skilled-nursing facilities evaluated by U.S. News, barely 2,000 earned a single High Performing rating in this year’s results. 

“We are honored to earn this national distinction for our short-term rehabilitation program,” said Nazareth Home President/CEO Mary Haynes. “This recognition illustrates the quality of services for patients on their path to successful rehabilitation.” 

As a result of the rating, Nazareth Home is eligible to display the U.S. News Best Nursing Home“badge” on their website, social media, advertisements, mailing, signage and other promotional media. 

Along with its healing environment, some of the short-term rehabilitation services provided by Nazareth Home include physical, occupational and speech therapy, private rehabilitation suites, state-of-the-art exercise technology and dedicated therapy gyms.

Nazareth Home names Melissa L. Bailey new director of communications

Nazareth Home is pleased to announce the appointment of Melissa L. Bailey as its new director of communications. Bailey will play an integral role in building a cohesive communications program for the long-term care and rehabilitation organization that encompasses branding, public relations, and employee communications. 

Bailey comes to Nazareth Home with more than 15 years of professional experience in the communications and marketing fields in both the for-profit and nonprofit sectors.

“We are excited to have Melissa join our team and continue to build on our strong history of person-centered care,” said Nazareth Home President/CEO Mary Haynes. “Her communication experience and skills will enhance Nazareth Home’s story in ways that connect and inspire people as we advance our mission of providing a healing ministry with a focus on hospitality.”

“I’m most excited to share wonderful stories about the lives being touched by the caring team at Nazareth Home,” said Bailey. “It’s an honor to be part of this great ministry that has such a rich history of serving elders and their families in the greater Louisville community.”

Bailey also volunteers for local cancer support groups and coordinates a respite care program for children living with special needs. She most recently served as the director of communications for the nonprofit Seven Counties Services / Bellewood & Brooklawn. Bailey is a graduate of the University of Kentucky.

Nazareth Home wins 2021 Best of Kentucky Award from KAHCF/KCAL

On Nov. 11, the Kentucky Association of Health Care Facilities/Kentucky Center for Assisted Living (KAHCF/KCAL) honored Nazareth Home as one of several premier long-term care facilities with a Best of Kentucky Nursing and Rehabilitation Award. Nazareth Home was honored to be a part of this elite group of leaders in the industry at the quality awards banquet. 

The Best of Kentucky – Nursing and Rehabilitation honorees were selected based on CMS star ratings of four or higher to apply, and then satisfaction surveys of residents and their families.

Along with a 2021 Best of Kentucky Award, three Nazareth Home employees received individual awards as follows:

      • Katrina Chesser, Unit Secretary and Certified Medication Technician, Highlands Campus – Professional Achievement/Caregiving Award 
      • Matt Elliott, Maintenance Supervisor, Clifton Campus – Supportive Care Award
      • Jason Rader, Environmental Services Director, Highlands Campus – Supportive Care Award

Long-term care providers attended the 2021 Annual Meeting & Expo, and were energized by general sessions challenging them with strategies to cope with stress on the job, hiring and retaining quality staff, COVID-19 safety measurements, and regulatory issues. On Nov. 10, KCAL held its KCAL Day for Assisted Living Communities. Also, more than 100 vendors exhibited during the Expo, showcasing the latest innovations in providing quality care.
